Reablement

Reablement Services
After experiencing illness or injury, you might find yourself in need of assistance and support to regain your independence within the comfort of your home. Our commitment extends to individuals seeking reablement services, especially after a hospital stay when support is crucial during the transition back to your home environment.
Page Hill Care reablement teams undergo specialized training equipped with skills tailored to facilitate your journey towards independence. Moreover, our team is adept at recognizing the emotional toll that illness or injury takes on you, as well as your family and friends. Our focus is not only on providing assistance but also on fostering the development of resilient coping mechanisms to navigate the emotional challenges that may arise during the reablement process. Our ultimate goal is to restore your confidence and independence as swiftly as possible during your recovery.

What care you’ll get
A team with a mix of people from the NHS and social services will help you do the things you need to do to stay independent. This might include getting dressed, preparing a meal, or getting up and down stairs. They might care for you at first, but will help you practise doing things on your own.
Your team might include:
- a nurse
- an occupational therapist
- a social worker
- carers
They’ll start with an assessment that looks at what you can do. You’ll agree together what you want to do and set out a plan.
The plan will include a contact person who’s in the team and the times and dates they’ll visit you.
